Already an Eyecare Associates of New Orleans patient? Ordering your lenses is easy! Simply fill out the form below. After submitting your request form, you will receive an email within 24 hours to complete your order and process payment. Please note submission of this form is NOT placing an order, you must follow the directions in the email to complete your order and process payment. If you reach out after 4 pm Friday through Sunday, you will receive an email the following Monday. Please check your spam folder for the link if you did not receive after 24 hours!
We appreciate your business! Please note that we supply replacement lenses for our own patients who have been examined by us within the past twelve months. If you are experiencing any problems with your lenses we suggest that you contact us to set up an appointment prior to ordering.
